Vagal Tone, HRV, and the Gut: How to Measure the Nervous System You Have

There is a measurement the conventional annual physical does not take. It changes daily. It responds to sleep, breath, food, conversation. It predicts more about how you will feel tomorrow than your cholesterol panel will. It is called heart rate variability.

HRV is the variation in time between heartbeats. A high HRV means the system is responsive, shifting moment to moment as conditions change. A low HRV means the system has locked into a pattern and cannot adapt. Most chronic illness lives at the low end of this scale.

HRV is a proxy for vagal tone, the activity of the vagus nerve. The vagus runs from the brainstem through every major organ and is the primary route the gut uses to talk to the brain. Roughly nine out of ten signals on this nerve travel up, from gut to brain, not down. When people say a gut feeling, they are describing literal nervous-system traffic.

The state of your gut shapes your vagal tone. The state of your vagal tone shapes how the rest of your body responds to stress, food, and rest. They are one circuit.

What raises vagal tone is specific. Slow extended exhales, particularly when the exhale is longer than the inhale. Cold water on the face. Singing, humming, chanting. Slow deliberate movement like qigong. A meal eaten slowly without anxiety. The shared meal at a table with people who feel safe.

What lowers it is also specific. Sustained vigilance. Eating while scrolling. Sleep at the wrong time. Caffeine on an empty stomach. The relationship that has not been honest for a long time.

The number on your watch is not the goal. It is a feedback loop. It tells you, day by day, what is working. The signal was always there. The instrument to read it is new.
